Waiver

I, the undersigned, hereby waive any and all claims against any and all persons associated with CheckMat Northridge Brazilian Jiu-Jitsu.

I understand I am participating in martial arts, essential to which is body contact and that such participation includes, but is by no means limited to the following: choke holds, arm locks, neck cranks, ankle locks, throws, slams, punches, kicks, elbows, knees, and the like, any one of which could potentially cause serious injury or even death. I recognize, moreover, that martial arts is an extremely dangerous sport and that minimizing the danger would defeat the purposes of the sport. My participation in all classes and events at CheckMat Northridge Brazilian Jiu-Jitsu, I hereby release CheckMat Northridge Brazilian Jiu-Jitsu and all of its agents, employees, associates and independent contractors of and from any and all liability, claims, demands, actions, medical bills, and other liabilities whatsoever arising out of or relating to any negligent act or omission, and / or any loss, damage, or injury, including death, that may be sustained by the undersigned and / or the undersigned’s personal property as a result of such participation.

I, the undersigned, also state that I am un good physical condition and know of no reason why I cannot participate in the physical activity. Accordingly, I assume full responsibility for any and all harm that may result to me as a consequence of my participation in the activities of CheckMat Northridge Brazilian Jiu-Jitsu and do accept the reality that is highly likely that I will at some point suffer some injury as a result of the same.

I authorize the use and disclosure of my name, photo-graphic / video images, and / or testimonial for marketing purposes by CheckMat Northridge Brazilian Jiu-Jitsu.

In signing the foregoing release, the undersigned hereby acknowledges and represents: that he / she has read the foregoing release, understands it and signs it voluntarily; that he / she is over 18 years of age and sound of mind
